New Patients:
New Patients are asked to arrive 15 minutes before your scheduled appointment time to fill out the necessary paperwork/forms. Please bring your insurance card/information. New Patient Form
Payment:
Payment is due in full on the day of service.
Senior Citizen Discount Available.
We accept Cash, Check, Visa, MasterCard, Discover and American Express.
We also accept Care Credit Medical/Dental card. This credit card is for Medical/Dental expenses only and offers up to one-year interest-free to pay the balance. Contact our office for more information.
Insurance:
We accept most insurances. Please provide your insurance information when scheduling your appointment to allow our office to verify benefits.
Dental Insurance is intended to cover some but not all of the costs of your dental care. Most plans include coinsurance provisions, a deductible, and certain other expenses which must be paid by the patient at the time of services. Reimbursement amounts are not, and never have been, a guideline for care. We work with you to ensure that you receive the maximum benefits to which you are entitled. We expect payment of any deductible/co-payments at the time of service.
